Periodontal Risk Score calculator

Score a periodontally involved tooth on seven factors and get its prognosis class with the published 5–10 and 15–20 year survival estimates. Based on the PRS validated on 6,762 teeth followed for a mean of 22.6 years (Saleh et al., J Periodontol 2023).

Score the tooth

Pick one option per factor. Points are the exact weights from the PRS card; the score updates as you go.

Age

Smoking

HbA1c

Non-diabetic patients score 0. Use the most recent value.

Tooth type

Mobility

Miller mobility class.

Probing depth

Deepest site on the tooth, in mm.

Furcation

Glickman class. Single-rooted teeth score 0.

In the validation cohort, a Class 2 tooth carried roughly 3.7× the hazard of periodontal tooth loss of a Class 1 tooth, and a Class 3 tooth roughly 11× (multivariate model adjusted for age, sex and maintenance; Harrell C-index 0.947). Scores above 11 are possible on the card but were rare in the study population; they fall in the Guarded class.
